Our organization has completed the first round of functional and technical surveys in order to calculate TIME.  Before we do our second round, we would like to vet our survey questions.  For those doing TIME, did you use the TIME survey template in LeanIX?  If not, how did you determine which questions to ask to gather functional and technical fit?  Did you create your own survey questions or did you work with a professional company to determine the survey questions? What other things did you do to gain confidence in the questions and that you were getting true functional and technical fit responses?

when we started with TIME there was nothing in place (so we did it using Azure Functions). Meanwhile we switched to calculated fields.
So we did a survey on our own. The time model is straight forward. We have split it up by different roles. We asked certain set of Questions to our technical subscriber (usually the IT-Administrator as Responsible) and our business subscriber (usually the BPO as observer). The functional fit was part of the survey to the business subscriber and the technical fit to the technical subscriber. For the technical fit we (manually) did a comparison between 6R (we have in place), technical fit and the calculated TIME value. So if 6R value is Retire and TIME value is invest, there is somethin going wrong.
Anyway from my Point of view you do not need consultancy (especially for the TIME approach), except it is the first time you to do a survey and you do not know how to, approach it (first send an email to the potential stakeholders telling them there is a survey coming, what it about and why it is important to participate). 
After finishing the survey we usually invite the stakeholders who not answered to an appointment and fill these fields in collaboration with them.

Btw.: TIME itself is usually easy, more complex is business criticality, because here you often have to discuss why some certain applications are not mission critical.

We are currently in the middle of our TIME evaluation. We don’t use the leanIX surveys for that.
For the technical evaluation, we have used the Gartner method. In short, we have listed, together with our infrastructure team, the list of technological platform used in our companies. The list looks like that:
- windows server standard,
- windows server non standard,
- linux server standard,
- …
- SaaS Standard
- SaaS non standard
- …
- windows desktop standard,
- windows desktop non standard.

Our infrastructure team has given a score (between 1 and 5) for each of those technological platform.

Regarding the functional fit, we created 2 surveys:
1 with questions we found on the net
1 with questions we created ourselves

The variety of our applications makes it difficult to find one universal questionnaire for all. So each IT team is free to use survey 1 or 2… and even, to chose a different format for exceptionnal cases, as long as they document how they got the score.

In all cases, the business owner + 2-3 key users are the ones filling the questionnaire that leads to the functional fit score.
